Welcome to the 2023-24 Orientation Day page. We are so excited for the new school year. Below you will find all of the forms and items you will need to complete for Orientation Day today.
Orientation Day Fees
All Orientation day fees are automatically billed to your Smart Tuition/Blackbaud Account. To see what fees were applied to your account, please click the button below.
Additionally, if you sign your child up for an extracurricular, before/aftercare, or athletics, payment will be processed through the tuition system.
Emergency Forms
Each year we require students have a new emergency contact form completed. Below is a button to the fillable Emergency Contact Form.
- Complete the form digitally and email it to sjshealthoffice@stjamesschoolah.org
- Or print the form, complete it by hand, and turn it into your homeroom teacher or the health office during Orientation Day.
Additional forms will be available during Orientation day at the Health Office table in the gym. An Emergency form is required to be completed for each of your children at SJS.
Medical Forms
The State of Illinois mandates a Certificate of Child Health Examination with Immunizations and/or requires a Proof of School Dental Examination and/or requires an Eye Examination Report upon entering certain grades.
- Certificate of Child Health Exam Form
Required for: PreK-3, Kindergarten, 6th grade, and all New students - Proof of Dental Exam Form
Required for: Kindergarten, 2nd, and 6th grade - Eye Examination Report
Required for: Kindergarten only - Sports Physical
Required for: Grades 4-8

Acknowledge/Agree to the Student/Parent Handbook
Please complete the form below acknowledging that you and your family will follow the policies fo the school as outlined in the Student/Parent Handbook. The Student/Parent Handbook can be found on the Current Families section of the website.
